Support Coordination

Our Support Coordinators assist, guide, and help the participants to be more independent in achieving their goals. Support Coordinator can help to find resources and services as per the need of participant to ensure the support provided meets the requirement.

Why Choose Us

Support Coordinator Role

Support Coordinator’s role is to help participants understand and make proper use of the supports available in the NDIS plan.

  • Connect participants with NDIS providers, community, mainstream, and other services
  • Build participants’ confidence and skills to use and coordinate your support.

Finding a Support Coordinator

Participants are able to choose both registered and unregistered support coordination providers. Support coordination can be delivered through in-person meetings, over the phone or via video based on participants’ preferences.

Support Coordination at Sagecare Australia

Sagecare Australia can assist participants to build the skills they need to understand and use their NDIS plan. Our support coordinator will work with participants to ensure a mix of supports are used to increase the capacity to maintain relationships, manage service delivery tasks, live more independently, and access the community.

Service Agreement & Pricing Arrangements

A service agreement between the service provider and participant is like any other agreement under Australian Consumer Law. The Service Agreement includes all details of support, charges, and service-related terms and conditions. We ensure each participant understands service agreement terms and has the same understanding of the type of support that will be delivered.

The service provided is charged as per the NDIS Pricing Arrangements and Price Limits (previously the NDIS Price Guide). The support prices charged are in accordance with the price guide. For further information on pricing and support, please discuss with our team about minimum charges that will apply for support.

Respecting Choice & Control

We understand the dilemma of finding the right person and we take pride in our services assuring the service is delivered with compassion and professionalism. We offer a range of services like Support Work, Domestic Assistance, Support Coordination, House Moving, Gardening and other, as required by the participant and give them choice and control over how they can achieve the goals as per their NDIS plan.
